Where to Obtain the Passport Application Form

Passport Form

Passport form can either be obtained online or offline:

  • Online: The passport seva portal provides access to online passport application forms that can be completed in one of two ways. One method involves downloading the electronic form, filling it out offline, and uploading it later at the provided URL (check the online application process). 
  • Offline: The second method involves filling out the electronic form offline and uploading it later at the site provided.  Additionally, there are two ways to access offline passport applications. 

Getting your passport form online is easier, quicker and more convenient than getting it offline. It should also be noted that going the offline route is only possible for new or fresh passport application forms. This is because the abovementioned centers (except PSKs) do not process any other type of application.

Where to Submit Your Indian Passport Application

Depending on the application method, passport application forms can be completed and submitted either online or offline. Print forms are submitted offline, while online forms are submitted online. On the passport website, online applications are made. 

The PSK and other collection facilities are where you can submit documents offline. Depending on how the forms are filed, the amount of the passport application fee and how the documents must be presented may also change. 

Passport Application Form Online

e-Form for Passport Application

You either fill them out and submit them online or download them first, fill them offline then upload them later. You can download the following e-forms from the passport website.

For applications processed at PSKs or regional passport offices (RPOs), e-forms have to be submitted online i.e. Printed versions of the same will not be accepted at these centers. Also, applicants must have at least version 9 of the acrobat reader to open any of these e-forms.

How to Print the Passport Application Form

The passport portal offers links to forms that may be downloaded and printed even though the e-forms can only be submitted online. They can be completed offline by handwriting in the information and physically turned in at the appropriate passport centre. These forms must be printed back-to-back on a4-sized paper.
The following printable forms are available for download from the passport website -

  • Main form for new or fresh/ renewal or reissue passport application
  • Supplementary form for new or fresh/ renewal or reissue passport application
  • Instructions booklet for the abovementioned main and supplementary forms
  • Forms (individual) for police clearance certificate, identity certificate and surrender certificate
  • Form for loc permit

As with the e-forms the applicants must have at least version 9 of the acrobat reader to open these forms.

Annexures and Affidavits

Affidavits, particular certifications, declarations, and particular letters, among others, are submitted with passport applications as annexures when necessary. There are several different annexures and/or affidavit kinds that can be accessed, downloaded, and printed, including -

  • Annexure A : This is the Identity Certificate.
  • Annexure B : This is a declaration by the parent or guardian of a minor for the minor's passport where one parent has not given consent.
  • Annexure C : This is a declaration by the parent or guardian of a minor for the minor's passport.
  • Annexure D : This is a specimen affidavit for submission to apply for a passport in place of a damaged or lost passport.
  • Annexure E : This is a no objection certificate issued by a ministry, department or office stating that it has no objection to the applicant getting the passport.
  • Annexure F : This is a prior intimation letter for submission by an applicant who works for the government, a public organisation or a statutory authority, informing the employer that the applicant is applying for a passport.
  • Annexure G : This is a declaration affirming the particulars furnished in the application about the minor.
  • Annexure H : This is a joint declaration in lieu of a marriage certificate

Authority letter :This is a letter of authorization that a passport applicant fills out to allow someone other than themselves to submit their application. It can also be used to give someone else permission to pick up the passport and any necessary paperwork. 

Appendix 12 : This lists the special cases of minors requiring passports.

Undertaking GSR 570 (e)

This statement of undertaking includes the applicant's name, parents' names, and spouse's name (if any), and is submitted on plain paper. Along with the pertinent passport file number or application reference number (ARN), it also contains the applicant's current living address.

Relevant information concerning the applicant's criminal history should also be provided.

FAQs on Passport Form

1.Can a Police Clearance Certificate be issued for more than one country through a single passport application form?

No, a Police Clearance Certificate (PCC) is issued for a single country through a single passport application form. You need to submit a separate PCC application for each country.

2.Will original documents have to be produced with the passport application form, or are certified copies enough?

You attach self-attested copies of the necessary documents to the passport application form and produce the originals at the counter. The originals are returned to you after verification.

3.When do Annexure A, G and H have to be submitted with the passport application form, and are they mandatory?

Government, public sector and statutory body employees applying for a passport must submit one of three documents with the passport application form: the Identity Certificate (Annexure A), the No Objection Certificate (Annexure G) or the Prior Intimation Letter (Annexure H). The document you submit decides whether the passport is issued without police verification, after police verification or on pre-police verification.

4.What documents does a government employee submit with the passport application form for a major change in name?

A government, public sector or statutory body employee applying for a major change in name submits a Gazette Notification changing the name and a fresh Identity Certificate in the changed name along with the passport application form. 5.Is pre-police verification possible when a government, PSU or statutory body employee changes their name on the passport form?

Yes, pre-police verification is possible when a government, PSU or statutory body employee applies for a name change through the passport application form. Post-police verification applies only to Tatkaal applications.

6.Can dependents of government, PSU or statutory body employees apply for a passport through the normal passport form procedure without affidavits?

Yes, dependents of government, PSU or statutory body employees can apply through the normal procedure, and the passport is issued without police verification if they submit the original Identity Certificate (Annexure A) of the employee. Otherwise, the passport is issued on pre-police verification, or post-police verification under the Tatkaal scheme.

7.Is a birth certificate mandatory with the passport application form?

A birth certificate is mandatory only for applicants born on or after the cut-off date in the Passport Rules. Applicants born before that date can submit a birth certificate or another accepted proof of date of birth, and the accepted list is available on the Passport Seva portal.

8.Do I need to submit a birth certificate with the passport form for reissue of a passport?

9.What is needed to change the date of birth on a passport through the passport application form?
